Is there a problem?
There are many self-administered tests to evaluate whether alcohol or drugs are causing physical, emotional or mental problems for a person. In determining whether a substance is being "used" or "abused", medical professionals consider each case individually. One or more of the following criteria are found in most people with addictive problems.
•Little or no control over the use of the substance (cannot stop at just one or two drinks, increased tolerance, needs additional/stronger substances to achieve desired effect).
•Preoccupation with the substance (avoids events where alcohol is not served, keeps a supply of substance on hand, maintains several sources to obtain the substance)
•Continues to use the substance despite adverse consequences (suspension/expulsion from school, warning or firing at job, DUI, family problems, financial problems).
•Distorted thinking (blaming self-caused problems on others, changing friends and associates to those who are more tolerant of addictive behavior, isolating and withdrawing from family and friends).
•Denial (can quit whenever they want, avoid others who point out their substance abuse, hiding quantities used from others).
Alcohol abuse is defined as more than 2 drinks a day for men, 1 drink a day for women, and/or more than 5 drinks in one session.
Prescription drug abuse includes taking more than the prescribed dosage, taking prescriptions written for others, and taking any prescription to achieve a desired effect other than the treatment of illness, disease or injury.
Other drug abuse is defined by laws as any use of illegal substances. Addiction is medically determined by the drug and its harmful physical, social and psychological consequences.
Help is available for everyone.
Admitting that an addiction problem exists is the first step towards becoming well. Sometimes the isolation from others makes it confusing to know where to turn. Help with conquering addictions is available for all ages and income levels. Help lines and rehabilitation centers are staffed by people who understand the battle, many of whom are also in recovery. (Recovery never ends!) The desire to quit the addiction is all that is required to open the door to a brighter, healthier future.

If help is needed, what happens next?
The goal of all recovery is to move people from addiction to freedom from the destructive substance. All people enter the recovery process at different stages and with different needs. Some people can successfully quit with the support of Alcoholics Anonymous or Narcotics Anonymous alone. Others may need to enter treatment, a supervised program of learning to live physically and mentally without the substance.
Assessment
Assessment determines the existence of a substance abuse or dependence by evaluating the physical, addictive and mental health issues. Assessments can be done in most treatment centers 24 hours a day. The assessment may include both a physical exam and a questionnaire regarding the substance(s) and the usage. A course of treatment will be recommended, which may include detoxification and an in-patient or outpatient program.
Detoxification
Abruptly stopping the use of many drugs and alcohol can be dangerous for those who are severely addicted. In these cases, the body needs detoxification or detox, to adapt to being deprived of the substance. Medically supervised detoxification should be undertaken by a physician led team of nurses and counselors. Medical complications from withdrawal must be monitored. Prescribed, physician administered medications can ease the discomfort of withdrawal from many substances. Detoxification is done in a hospital or clinic, and may take several days to several weeks.
Residential/In-Patient Programs
Patients stay in the hospital or center for a prescribed period of time (from a several weeks to several months) in a supportive, structured program. Steps to recovery (often a 12 step program) are reviewed. Personal and social issues leading to the addictive behavior and strategies for preventing relapse (return to drinking or use) are taught. In addition to needed medical supervision, support is given through individual and group counseling.
Out-Patient Programs
Patients are required to attend counseling on a daily or several times weekly basis. Education is given on recovery strategies such as Alcoholic Anonymous or SMART Recovery®. Patients often meet with others in the recovery process, and are given resources and contacts to assist in their daily recovery.
Cost of Treatment
Recovery resources are available for everyone. Community based programs and Alcoholic Anonymous are free. Sliding scales based upon ability to pay are offered at many hospitals and some treatment centers. In-patient services cost more, and may be very expensive. In-patient services which are medically necessary due to advanced addiction may be covered by medical insurance.
