Lets see, Chief Webber steps down, Dr. Shepherd gets offered the position turns it down, then its given to Dr. Torres by default. Izzie tells George how she feels about him, then finds out through Callie that George and her are talking babies, which just so happens to be at Dr. Yang and Dr. Burke's wedding which falls out after Christina "thinks" she's ready to be married and Preston isn't satisfied, clears his things out of the apartment and is gone.
Dr. O'Malley is no longer a doctor after he fails the internship and basically tells Dr. Bailey (Who should be Chief Resident) that he doesn't want to start over, then of course dealing with his feelings for both Callie and Izzie (which he looks spaced out) while Bailey is simply a work-a-holic trying to understand life after Chief Webber stepping down and where her clinic is headed.
Dr. Karev has pretty much took himself out of any love spats right before his internship exam, Dr. Walsh is moved to sunnier days in the new spin-off of Grey's Anatomy, Dr. Sloan......well you never know what exactly is going on with him so we'll just leave that one alone, which is probably for the better. Dr. Shepherd and Dr. Grey have seen there love affair become stagnant and unsure of what steps to take next, not to mention the emotional roller coaster that Meredith is on in every episode. Whew~! Okay, so now that your caught up.
Season 4 was as frantic an episode as we've seen to this point. Yang and Grey come back from a honeymoon together without Burke of course, all the interns from Season 3 are now residents in Season 4 with their own interns in which not only George is back in the saddle but a newcomer, Meredith's sister is one as well. Callie has absolutely no idea what she's doing as Chief resident and is as nervous as can be, Dr. Shepherd is now trying to make friends within the hospital, and Izzie is getting rumored about her past performances by her interns. Did I mention this was all within the first 5 minutes?
Kind of makes you feel like its your first day at a new job, and you know what to do but the company you are starting for has a different way of doing things and you feel all out of sorts, well thats this episode! An accident occurs and Izzie sees blood on a young boy who says "she's hurt" and that propels Izzie to believe another person is hurt in this accident but it turns out to be the deer that caused the crash in the first place which was a bit cheesy, but definitely something Izzie would eventually try to help since she is shown as having a huge heart on the show. I said it was Cheesy didn't I, oh wait I did okay.
Yang finally realizes that Burke is gone for good when her and Dr. Shepherd are prying about each other's love interests and Derek tells her Preston put in his letter of resignation 2 weeks prior. Finally Izzie and O'Malley talk about the distance that have built up between them and George just flat out blows her off. Mark tries to buddy up with Derek and bad mouth Meredith, but the two of them just seem that they will never get along on air.
On a serious note medical wise there is a pregnant woman that has an arm severed, a man whose head is detached from his spine, another man who thinks its cool to eat everything from paper holders to scissors and everything else he can get his hands on. Then of course Izzie wound up saving the deer by jump starting her heart right in the back of a pick-up truck outside the hospital.
While many of the interns seemed more like extras, Meredith's half-sister was had a prominent role in this episode which would lead viewers to believe she will be around and play a more important role down the line. As the first episode of the new season winds down its safe to say many story lines will be interacted and overlapping one another , omg lol, okay last but not least Mark professes his love and real reason he came to Seattle and that was to get Dr. Shepherd back as a friend, Izzie sticks up for herself to what the interns said to about her earlier, O'Malley delivered a baby on his "first day" so Lexi told them, and if you want to know the rest you can view the episode online here for an ending you wouldn't have expected this early on.
