1. Project Declaration and Plan: State What You Would Like To Do. Address the Following:

a. Reasons for Change
b. What You Hope To Learn/Achieve and Why
c. Prior Experiences in making Change?
d. Start and Stop Dates
e. Ways To Determine Progress (milestones, checkpoints, etc)
f. Project Plan: Develop and describe a plan for carrying out your behavior change project. Describe what you plan to do and the ways you will determine your progress.

2. Research: List Out Resources You Wish to Refer To when Doing This Health Behavior Change

3. Weekly Log Segments '" Noting Any Obstacles You Face Especially to Give Special Attention on

4. Assessment: Write an Evaluation of your experience by:
a. Describe the Experience of trying to Accomplish Your Goal
b. List Two Aspects about your topic that you learned
c. Described what the project help you to learn about yourself and how to change a health behavior
d. Was the Project Worthwhile and Why?
