Obviously be warned there is an entire episode spoiler ahead.
House MD season six, episode four "Instant Karma" begins with an introduction to Mr. Randal the teenage patient of episode four's focus's father. Mr. Randal is the head CEO of an inherited pipeline business. His son is suffering from abdominal pain and appears to be dying. The best doctors' money can by fail to cure the patient so Mr. Randal seeks out good doctors, but the best doctor, Gregory House. Cuddy reluctantly let's House call all the shots on the case while Foreman pretends to still be in charge.
Meanwhile Chase and Foreman are stressing over the recent death of the foreign president in House MD episode three, "The Tyrant". His death is under investigation and as we know Chase in effect murdered the dirty dignitary. A discrepancy in cholesterol levels is discovered by Foreman between early blood tests and the test that Chase faked to secure the wrong diagnosis. The two spend the entire House MD season six, episode "Instant Karma" sneaking around to discuss how to cover their lies.
The team examines the sick teen and finds his bowel to be nearly impacted. They believe at first this is a case of simple constipation. An enema is administered and at first the boy appears to be better. Moments later he has a seizure due to increased cranial pressure and it's clear the diagnosis is incorrect. Mr. Randal then reveals his theory that his success in business is tipping the balance of karma. He believes not all aspects of his life can be perfect and that the sacrifice for his millions will be his family.
Clearly House disagrees. Next the teenage boy begins to get spots all over his body which leads House and the team to believe the boy has an incurable disease that will end in his death. The boy's father decides to initially bankrupt his company in an effort to make karma spare his son.
Doctor Thirteen remains off the team in episode four of House MD season six and is planning to fly to Thailand. House does not think this is a good idea and cancels her first tickets. Wilson later admits to being the one to cancel the tickets, but only to protect House.
As House and Wilson are discussing her leaving House has an epiphany and realizes he has misdiagnosed the boy. He administers the cure and the boy recovers, unfortunately his father has already succeeded in bankrupting his company and still believes that karma was the cause of his son's ailment. An unusual occurrence in House MD season six, Episode four "Instant Karma" is that House makes no effort to convince the father otherwise. He lets it go.
Episode four of House MD season six ends as Foreman and Chase are relieved to find an answer to why the President's Cholesterol was wrong, provided by House anonymously which Foreman presents at the investigation and Thirteen flies away to Thailand.
