You can get creative and have drawings or even report cards. Any information that is important to you, is great to include.

Next you want to work backwards and put the parents of this person. If you are doing this yourself you can nicely write all the information you know about these people. You can go to a library and look up information about family history. You can also find out a lot of information about your ancestors on-line.
