First, understand your strengths.
There are a ton of assessment tests available online (and offline), though most require a fee. One site that offers a free test is found at http://www.testingroom.com. And if it's been awhile since you've taken a test, now might be a good time to take a more recent test and to have those results on hand. Strengths can and do change over time with life experiences. These may draw out new strengths developed through anything from starting a family to taking on new sports or even community involvement.
Reflecting on your strengths, think about what you want to work on in the mentoring relationship. Try to figure out what goals you personally hope to achieve and challenges you'd like to work through. This helps you and the mentor shape the direction of the mentorship and forms a basis for measurable results. Try to write out at least 3 goals.
Ask.
Pick 3 people you'd like to have mentor you. Your selections will just depend on what your needs and goals are for the moment. Before you ask, do a gut check to make sure you are indeed open to feedback and that you are coachable. Sometimes your mentor will not be helpful if you cannot (or won't) follow through on the advice given for whatever reason.
When approaching a possible mentor, check to see if the person is open to the idea of mentoring. Make the requests in person. Let each person know that you admire her/his skills and accomplishments. Be flattering without fawning. Just say that you'd like to learn more about how they've achieved their current success level.

And be sure to ask about their availability. Check to see if they have time to mentor. If they don't have the time, thank them and move on. Keep the door open by asking if you may put in an additional request at a more appropriate time. You don't want to overload a popular mentor.
Start the Relationship.
When the person agrees, begin to set up a protocol. Make it easy on the mentor by offering to set meeting times and dates. Together, pre-determine the length of time of the relationship. Be flexible and committed. Show up. Ask that all conversations be kept confidential on both sides. Let them know upfront that you will respect anything they share.
Throughout the mentoring process, put in work on your own, and be proactive in assisting in working through your own issues. Collaborate closely with the mentor and follow through on feedback and suggestions. Remain open and candid. Having a mentor that is closer in rank or standing to you will help facility the ability to deepen the relationship.
Ending the relationship.
Celebrate the ending of the relationship by taking them to lunch/breakfast. This is a smooth, easy way towards conclusion. Use this opportunity to communicate what you felt you have achieved and how much you've enjoyed and appreciated their help.
Continue this habit of actively seeking and working with a mentor you'll soon have a verifiable track record of improvement. These relationships can form a foundation of champions to cheer you on to other advantageous contacts or positions.
