I encourage you to always express concerns, ask questions, or offer suggestions; this is just another way to help me understand yours and your child's needs so that I can provide better care for your child. Please answer truthfully; positive answers are flattering but unhelpful if dishonest. Please use the back of the evaluation to expand your answers, ask questions, request information, or anything else you may wish to add to this evaluation. And remember, the more information you can provide, the better I can provide care that is tailored to your specific needs.
