Pet health insurance was the best decision for you and your family as your pet is taken care of and your finances take less of a blow. You knew that it would be worth it to have money sitting somewhere for an unexpected illness or injury and now your policy is about to pay off. But how do you file a claim on your pet's health insurance? It is actually quite easy, here is the information you need on almost every pet insurance claim form.
  1. Your Personal Information: It is important that the pet insurance company have your personal information to store the information safely. Pets don't have things like social security numbers so it becomes tough to store their information. There are only so many ways to sort through all of those "Fidos" out there!
  2. Your Veterinarian's Information: Usually all the information that is required about your vet is their name, address and phone number. Most pet insurance companies do not require you to go to a specific vet to get service for your pet. However, this is a good way for insurance companies to protect against fraudulent claims.
  3. Your Pet's Information: As silly as it may seem to write "Mr. Marbles" on a legal document, you must fill out your pet's name, age and gender. In addition it would likely be necessary to mark what type of pet this is (dog, cat, bird, etc.) and the breed of the pet. All of this helps track and classify the claims that are being filed.
  4. Your Insurance Information: The insurance company will likely want to know what type of policy plan you have and the policy number you are filing under. This information will help expedite the claim to keep it from getting lost in the shuffle.
  5. The Nature of the Condition (Diagnosis): This field is very important and may need to be filled out by the vet him or herself. The date of the treatment and the amount charged will also be needed in addition to possible follow up questions that relate to this possibly being a pre-existing condition and if this is your pet's usual veterinarian.

Some insurance companies may have additional questions, but these questions are standard on almost all pet health insurance claim forms. Remember that the more detail you can provide the better and that a completely filled out claim form will be processed quicker.
