Their relationship in the sixth season of 'Supernatural' has been very strained to say the least. Dean found out that Sam came back from hell and didn't tell him for a long time afterward. Dean was understandably upset about that as he was torn apart by his brother being in hell with the devil himself.
As Dean started to work with Sam again, he got the feeling that something wasn't quite right especially after Sam watched Dean get bitten by a vampire. It turns out that an important part of Sam was missing, the piece that made Sam, Sam. His soul. Without his soul, Sam was just a body functioning, not caring what was right or wrong and in some cases not able to tell what was right or wrong.
Dean tried to accept Sam while he strove to find a way to get Sam's soul back but every bad choice that Sam made, Dean had a harder time being around him. Sam started fighting Dean every step of the way because he wasn't sure he wanted his pesky old soul back that would allow him to make what he thought were stupid decisions and to feel things that he didn't want to feel.
Sam came up with a plan to kill Bobby, a man who has been like his father and another hunter, while Dean was off spending a day in the life of the Grim Reaper. Bobby figured that Sam was up to something though and thwarted his plan as best he could but ultimately was caught and almost killed until Dean came and saved him at the last minute.
Dean had found out from the Death that Death could get Sam's soul and put it back but there would be consequences. Sam's soul had been in a cage being toyed and played with by Lucifer and Michael. If Death put the soul back in, Sam could go insane. Death said that he could put up some sort of a mind block to hold some of that back but that it could end up breaking and he didn't know when that would happen. Dean decided it was worth the risk instead of having a soulless brother who would kill at a whim.
So now Sam has his soul back but whether he will survive the ordeal remains to be seen. Even if he is OK in the beginning, the mind block could break at any time. Sam and Dean's relationship may not be able to survive. Dean might not be able to forget the soulless Sam and Sam might not be able to forgive Dean for taking the risk of putting his soul back in.
