Though not shuttle services, the New York Airport Service Express Bus, Airlink New York, and Trans-Bridge Bus Lines offer service to limited destinations in Manhattan from Kennedy Airport.

ETS Air Shuttle has been transporting people to and from Kennedy Airport since 1996. They offer door-to-door shared rides from Kennedy Airport to Queens, the Bronx, and Brooklyn for approximated $35 per person and $10 for each additional person. Long Island & New York State
Shuttle service between Kennedy Airport and Long Island is scant. A company called Classic Airport Shuttle was providing service. However, according to a June 12, 2008 SuperShuttle Press Release, SuperShuttle acquired Classic Airport Shuttle. The best bet is to check with SuperShuttle to see if it is currently providing service to your Long Island location.
Winston Transportation Group also offers shared ride and limousine service between Kennedy Airport and Long Island.
Otherwise, the Long Island Rail Road (LIRR) and taxicabs are additional options. I use the LIRR several times a year and it is not a bad ride. It is comfortable, safe, air conditioned and well-attended. However, pre-arranged transportation from the Long Island station to your final destination is necessary. Taxicabs are available but they can be a bit pricey. Also, some station platforms can be rather deserted at night which is something to keep in mind if you are traveling alone. Do keep an eye on the gap between the train and the platform. Some of them are rather wide; injury and death have occurred.
For people traveling to and from Kennedy Airport and New York State, Westchester Express services Westchester, Rockland & Putnam Counties. It offers shared door-to-door shuttle service which can be less expensive than a taxi. For those who would like to splurge, Westchester Express also offers private limousines and luxury sedans.
Of course, one of the ultimate splurges in shuttle services to and from Kennedy Airport is Delta's US Helicopter Airport Shuttle Service. It takes Delta passengers to and from Kennedy Airport and Manhattan in a mere 8 minutes for $159, one-way. Complimentary rides are available based upon Delta flight arrangements. There is nothing like a ride in a whirlybird. Any adventurous soul will attest to that.
